What type of information we have
We process data that you provide to us through our contact forms. This includes:
- Contact details
- Date of birth
- Reason for referral
- Background information i.e. medical history, family and traumatic events
We also collect minimal anonymised data regarding visits to our website (e.g. whether a first time or repeat visitor).
If you submit a form on our site you may opt in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you submit another form. These cookies will last for one year.
How we get the information and why we have it
We collect information about you and your client when you voluntarily complete our contact forms.
The legal basis for processing any data that you provide to us is based on contract, that is to provide you with an accurate recommendation of assessment/treatment, and the associated costs.
What we do with the information we have
We will only contact you for marketing purposes by email when you have provided your explicit consent in order for us to do so, by way of opt in on our contact forms. You can opt out at anytime by emailing firstname.lastname@example.org
How we store your information
All personal and sensitive information is stored securely on our Onedrive in compliance with EU General Data Protection Regulations (GDPR) rules.
We keep enquiries for a period of one year after the date of contact. We will then dispose of your information by permanently erasing it from our Onedrive.
